About the Patient Portal:

New Patients will receive an email with a username and temporary password to access the Patient Portal.

This Patient Portal will provide a secure website for new patients to provide their patient history through an online patient questionnaire prior to coming in for their first appointment.

If you are unable to complete the patient questionnaire online prior to your visit, please arrive 20-30 minutes early for your scheduled appointment to complete this information in our office.
